Learn how to create a form:
- Navigate to your "Form builder" page, under Settings, or click here.
- Click on "Create a new form"
- Enter the name of your form. This form name will be publicly visible to your visitors who fill out this form.
- Optional: Include a description below for your form. This description will also be publicly visible to your visitors.
- Click on "Add a field to this form" and select an existing LACRM field to add to your form.
- Once you have added all the fields you need for your form, click on 'View form' on the top right corner to see the public version of your form.
- Note: What makes LACRM Forms different from other form tools you may already be used to is that the you are adding existing LACRM fields to your form, instead of creating form fields and then linking them to LACRM later. You can change the name of a field in your form to make it more relevant to that specific form. For example, instead of asking your visitors to select a dropdown option for a "Source" field, you can rename that field to "How did you hear about us?" for just this form. Your visitor will then select from your existing "Source" field options, and what they submit will populate your "Source" field.
